Booking your first visit
We consult from Suite 302, St John of God Subiaco Clinic, 25 McCourt St, Subiaco.
Schedule an initial consultation by:
- phoning our rooms at (08) 6270 0123, or
- sending an online request for an appointment HERE
If booking an antenatal first visit, we would like to see you between 8 to 10 weeks pregnancy.
Urgent appointments
Please advise our staff if the matter is urgent and they will discuss it with one of our doctors. We will endeavour to arrange an expedited appointment if indicated.
Before your visit
Please download, fill out and email or bring the New Patient Form for either Obstetrics or Gynaecology with you to your first visit.
Referral information
Anyone can make an appointment to see our doctors.
Valid referral
To claim a Medicare rebate patients must have a valid referral that is less than 12 months old if it is from a General Practitioner, Eligible Nurse or Midwife or 3 months if it is from another specialist.
No valid referral
Without a valid referral you do not qualify for a Medicare rebate and will have to pay the full fee out of your own pocket.
You can make an appointment and then arrange for your general practitioner to fax through a referral prior to your appointment date.
